Job Title: Case Manager RN

Job Summary:

We are seeking a highly motivated and organized Registered Nurse to join our Case Management team. The successful candidate will be responsible for assessing, planning, implementing, and coordinating all case management activities with members to evaluate their medical needs and facilitate their overall wellness.

Key Responsibilities:
  • Conduct thorough assessments of members' medical needs and develop proactive plans to address issues presented.
  • Collaborate with members, healthcare providers, and other stakeholders to ensure seamless care coordination and optimal health outcomes.
  • Utilize clinical judgment to incorporate strategies designed to reduce risk factors and barriers, and address complex health and social indicators that impact care planning and resolution of member issues.
  • Apply knowledge of clinical tools and information/data review to facilitate integrative functions and smooth transitions to Aetna programs and plans.
  • Consult with supervisor and others to overcome barriers in meeting goals and objectives, and present cases at case conferences for multidisciplinary focus to benefit overall claim management.
Requirements:
  • Active and unrestricted RN licensure in the state of Ohio.
  • 2+ years of clinical experience as an RN.
  • Ability to travel up to 50% - 75% in Franklin, Delaware, Madison, Pickaway, or Union counties.
  • Reliable transportation required, with mileage reimbursed per company expense reimbursement policy.
  • 1+ year of experience with the Microsoft Office suite.
Preferred Qualifications:
  • Home Health experience.
  • Computer skills, including Microsoft Office (Word, Excel, and Outlook).
