Job Summary:

We are seeking a skilled Physical Therapist to join our Home Health team as a Rehabilitation Home Care Coordinator. As a key member of our team, you will play a critical role in providing exceptional patient-centered care to our patients in the comfort of their own homes.

Responsibilities:

  • Manage a caseload of patients and provide physical therapy services to meet their individual needs.
  • Develop and implement personalized treatment plans to help patients achieve their goals and improve their quality of life.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families to promote independence and self-care.

Requirements:

  • Valid license to practice physical therapy in the state of Idaho.
  • 2 years of relevant experience in home health or a related field.
  • Current Basic Life Support (BLS) certification.
