Job Overview

We are seeking a highly skilled Home Health Therapy Clinical Manager - Physical Therapist (PT) to join our team. This role will involve overseeing the clinical operations of our home health services, ensuring high-quality patient care and outcomes.

Key Responsibilities:
  • Ongoing Patient Assessment: Ensure ongoing interdisciplinary assessment of patients to identify their needs and develop effective care plans.
  • Clinical Oversight: Support and oversee home care clinicians to ensure they meet and exceed standards for safe, effective, and efficient clinical practice.
  • Team Supervision: Supervise paraprofessional and licensed professional team members to promote teamwork and collaboration.
  • Clinical Decision Making: Use clinical decision-making skills to efficiently and effectively manage an assigned patient population.
  • Patient Education: Develop education programs for patients and their families to promote progress toward mutually established goals.
Requirements
  • Educational Background: Graduate of an accredited Physical Therapist Program and active PT license in Colorado.
  • Experience: Minimum of three years of physical therapy experience in home healthcare preferred, with two years of management experience also preferred.
  • OASIS Experience: Two years of OASIS experience preferred, with ability to conduct extensive OASIS documentation and chart audits.
  • Certifications: CPR Certification required.
  • Computer Proficiency: Must be computer proficient and able to work independently, providing effective communication to patient/family, team members, and other healthcare professionals.
  • Regulatory Knowledge: Knowledge of state and federal home health regulations.
