About the Role

At Frontier Home Health and Hospice, we're seeking a dedicated and compassionate Registered Nurse (RN) to join our team in a PRN capacity. As a Hospice Nurse, you'll have the opportunity to make a meaningful impact in the lives of our patients and their families.

Responsibilities
  • Provide high-quality patient care in private homes, residential care facilities, and other hospice care environments.
  • Work collaboratively with patients, families, and other healthcare professionals to develop and implement individualized care plans.
  • Supervise licensed vocational/practical nurses and nursing aides as needed.
  • Conduct thorough assessments, identify patient needs, and develop effective care strategies.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • Current RN licensure in the state of employment.
  • Valid state driver's license, automobile liability insurance, and dependable transportation.
  • Minimum of one year of clinical experience, with home health or hospice experience preferred.
