Hospice Clinical Liaison Nurse Specialist

Envision Hospice is seeking an experienced and empathetic Registered Nurse (RN) to serve as a Hospice Clinical Liaison. This role will act as a liaison with hospitals, facilities, physicians, and other referral sources to promote effective relationships, communication, and problem-solving. The successful candidate will provide assessment, supportive care, and education to prospective patients and families regarding hospice services.
Key Responsibilities- Assist Account Executives and Clinical Leadership in clinical care coordination of referrals, patients, families, and facilities.
- Assess and counsel patients and families during initial hospice discussions to determine the level of care needed and hospice eligibility factors.
- Be a subject matter expert on hospice eligibility criteria.
- Complete admission and consent paperwork efficiently and accurately, demonstrating knowledge of the admission protocol.
- Ensure all required information and paperwork is provided to patients and families, answering questions and ensuring understanding of hospice information.
- Provide detailed explanations of hospice services and benefits to patients, families, and referral sources.
- Assess and place orders for medication needs, overseeing the placement of orders for durable medical equipment, supplies, and oxygen as needed.
- Address, communicate, and assist in all resolutions.
- Provide discharge planning assistance from hospitals or facilities as needed.
- Consult with hospital staff and/or facility staff to determine discharge orders and facilitate discharge to home.
- Interface with referring physicians, nurses, ancillary providers, and discharge planners to facilitate a well-coordinated transition to hospice for patients and families.
- Communicate pertinent patient/family information to the interdisciplinary team and follow through to facilitate a smooth transition.
- Develop and maintain professional relationships with referral sources, including attending meetings and participating in community events to promote hospice services.
- Document all patient interactions, referrals, and outcomes in the hospice's EMR system to ensure accurate tracking and follow-up.
- Train and support new staff on hospice liaison role, including best practices for patient communication and referral source management.
- Participate in interdisciplinary team meetings as needed.
- Ensure patient advocacy in all aspects of the role and represent Envision Hospice in a professional manner at all times.
- Must have sales experience.
- BA or BS degree preferred, must have an associate's degree in nursing or be a graduate of an accredited RN program.
- Must have a license as a RN in the state of Washington.
- Minimum of 1-2 years of experience in hospice, home health, and/or palliative care.
- Effective communication abilities, including strong verbal and written group presentation skills.
- Employees must be computer literate, have a reliable car, valid driver's license, current car insurance, and a good driving record.
