Occupational Therapist Job Description

We are seeking a skilled and compassionate Occupational Therapist to join our home health care team at Haven HHC. The ideal candidate will provide home visits to patients in the Sedona area, helping them regain independence and improve their quality of life through customized therapeutic interventions.

Key Responsibilities:
  • Conduct in-home occupational therapy assessments and develop personalized treatment plans to enhance patients' daily living skills and functional abilities.
  • Collaborate with the interdisciplinary healthcare team to ensure coordinated care for each patient, promoting a seamless and effective treatment experience.
  • Provide education and training to patients and their families on adaptive equipment, home modifications, and therapeutic exercises to support their recovery and independence.
  • Maintain accurate and up-to-date documentation of all therapy sessions, assessments, and patient progress to ensure continuity of care.
  • Participate in case conferences with other home health team members to discuss patient care and improvements, ensuring a comprehensive and patient-centered approach.
Qualifications:
  • Bachelor's or Master's Degree in Occupational Therapy from an accredited program.
  • Current licensure to practice as an Occupational Therapist in the state of Arizona.
  • Minimum of 1-year experience as an Occupational Therapist, preferably in a home health or healthcare setting.
  • Strong knowledge of therapeutic techniques, adaptive equipment, and patient-centered care.
  • Excellent communication, organizational, and time-management skills.
