Responsibilities- Provide Direct Patient Care: Assess patient needs, develop and implement individualized care plans, and provide ongoing monitoring and evaluation of patient progress.
- Collaborate with Healthcare Team: Work closely with physicians, nurses, and other healthcare professionals to ensure comprehensive and coordinated care.
- Conduct Health Education: Educate patients and their families on healthy habits, disease prevention, and management of chronic conditions.
- Manage Patient Records: Maintain accurate and up-to-date patient records, including medical histories, test results, and treatment plans.
- Participate in Quality Improvement: Contribute to quality improvement initiatives and participate in ongoing education and training to stay current with best practices.
- Education: Graduate of an accredited nursing program and a Master's degree in Nursing or a related field.
- Experience: At least 2 years of experience as a Nurse Practitioner in a healthcare setting.
- Licensure: Current licensure as a Registered Nurse and certification as a Nurse Practitioner in the state of Oklahoma.
