Job Summary

We are seeking a skilled and compassionate LVN Charge Nurse to join our team at Windsor Nursing and Rehabilitation Center of Weslaco. As a key member of our nursing staff, you will play a vital role in ensuring the highest quality care for our residents.

Key Responsibilities
  • Assist in the development and implementation of individualized care plans that promote the physical, mental, and psychological well-being of each resident.
  • Conduct regular rounds to assess resident condition, document changes, and report to the healthcare team.
  • Administer medications as prescribed and oversee the implementation of care plans.
  • Collaborate with the physician to ensure accurate and timely documentation of resident visits and care.
  • Perform narcotic reconciliation and identify significant changes in resident condition, taking necessary action per facility policies and procedures.
  • Supervise and monitor aides to ensure high-quality care delivery.
  • Initiate and complete incident and accident reports as needed.
  • Communicate effectively with residents, families, and healthcare team members to ensure seamless care coordination.
Requirements
  • Active Licensed Vocational Nurse in the state of Texas in good standing.
  • Experience working in a skilled nursing facility is preferred.
