Job Summary

We are seeking a compassionate and motivated Physical Therapist to join our team. As a Physical Therapist at JAG Physical Therapy, you will have the opportunity to work with patients in their homes, providing one-on-one care and developing personalized treatment plans to help them achieve their goals.

Responsibilities
  • Collect and record patient care data, including initial evaluations, daily notes, reassessments, and discharge plans
  • Administer tests and measures, and synthesize evaluation results to formulate diagnoses, goals, and plans of care
  • Establish and implement treatment plans, collaborating with interdisciplinary teams and patients
  • Provide education and training to patients and their support systems
  • Conduct functional training, including ADLs, IADLs, and therapeutic activities
  • Prescribe and train patients on rehabilitation equipment
  • Provide interventions, including musculoskeletal interventions, and modalities
  • Document patient data and treatment plans using our EMR system
Requirements
  • Bachelor's or Master's degree in Physical Therapy
  • Licensed Physical Therapist in the state of practice
  • Completed state-regulated CEU requirements
  • Valid CPR license
  • Experience with EMR software preferred
  • Excellent communication and clinical decision-making skills
