Job Summary

The Rensselaer Care Center is seeking a skilled and compassionate Care Coordinator to join our team. As a key member of our nursing staff, you will be responsible for providing exceptional patient care and ensuring the highest level of service to our residents.

Key Responsibilities
  • Supervise and Coordinate Patient Care: Oversee the daily functions of unit nurses and CNAs, providing leadership and guidance to ensure seamless patient care.
  • Charting and Documentation: Accurately and timely document patient information, medication administration, and other relevant data.
  • Medication Administration: Prepare and administer medication as ordered by a physician, ensuring patient safety and well-being.
  • Emergency Response: Respond promptly and effectively to emergency situations, prioritizing patient and staff safety.
  • Interdisciplinary Collaboration: Communicate and work collaboratively with healthcare teams to coordinate patient care plans and services.
Requirements
  • Nursing Diploma or Degree: Possess a current RN license in good standing and maintain active licensure throughout employment.
  • Geriatric Nursing Experience: Preferably one year of experience in geriatric nursing, with a strong understanding of patient care needs and best practices.
  • CPR Certification: Hold current CPR certification and maintain it throughout employment.
