Job Summary:

We are seeking a skilled Occupational Therapist to join our team in providing high-quality care to patients in their homes. As a Home Health Occupational Therapist, you will be responsible for assessing and evaluating patient care needs, developing and implementing individualized therapy plans, and providing ongoing support and education to patients and their families.

Responsibilities:

  • Evaluate patient's functional status and occupational therapy needs
  • Develop and implement individualized therapy plans
  • Provide ongoing support and education to patients and their families
  • Collaborate with healthcare team to ensure comprehensive care

Requirements:

  • Current Occupational Therapy licensure in state of practice
  • Current CPR certification
  • Current driver's license, vehicle insurance, and access to a dependable vehicle
