Overview

The Laurels of Hamilton is a 80-bed skilled nursing facility seeking a full-time MDS Nurse to complete our clinical leadership team.

Responsibilities
  • Conducts assessments and care plan coordination for residents assigned.
  • Completes the MDS, CAA's, and care plans within regulated time frames.
  • Assesses residents through physical assessment, interview, and chart review.
  • Discusses resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff.
  • Reviews information from hospitals, consults, and outside agencies, and uses such information in the completion of assessments and care planning.
Requirements
  • Registered Nurse (RN)
  • Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment Process (PPS), strongly preferred.
  • Experience as an MDS Nurse, preferred, but willing to teach.
