Advocate Aurora Health is seeking a skilled Occupational Therapist to join our Home Health team in Justice, Illinois. As a key member of our interdisciplinary care team, you will provide personalized home-based therapy services to patients, promoting optimal health outcomes and improving quality of life.
About the Role- We are offering an estimated salary range of $85,000 - $105,000 per year, commensurate with experience.

- As an Occupational Therapist with Advocate Aurora Health, you will work collaboratively with patients, families, and healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.
- You will have the opportunity to work with a variety of populations, including adults and children, and will be supported by a multidisciplinary team of healthcare professionals.
- Conduct thorough assessments of patients' physical and cognitive abilities, identifying areas for improvement and developing targeted interventions to address these needs.
- Develop and implement effective treatment plans that promote functional independence, mobility, and overall well-being.
- Collaborate with patients, families, and healthcare teams to ensure seamless communication and coordination of care.
- Maintain accurate and timely documentation of patient progress, adhering to all relevant laws and regulations.
- Participate in ongoing education and professional development opportunities to stay current on best practices and evidence-based therapies.
- A graduate degree in Occupational Therapy from an accredited program.
- Current licensure or certification as an Occupational Therapist (OTR/L) in the state of Illinois.
- Ability to lift up to 35 pounds without assistance, with use of patient handling equipment for heavier lifts.
- Excellent communication and interpersonal skills, with ability to work effectively with diverse populations.
- Proficient computer skills, including Microsoft Office and electronic medical records.
