Care Coordination RN - Registered Nurse Case Manager
Job Summary:As a Care Coordination RN - Registered Nurse Case Manager, you will play a vital role in assessing, planning, implementing, coordinating, monitoring, and evaluating options and services to optimize the patient or member's health status. You will manage utilization and practice metrics to refine the delivery of care and maximize clinical, quality, and fiscal outcomes.
Key Responsibilities:- Integrate evidence-based clinical guidelines, preventive guidelines, protocols, and metrics into treatment plans that are patient-centric, promoting quality and efficiency in healthcare delivery.
- Develop systems of care that monitor progress and promote early intervention in acute care situations.
- Assist in designing, implementing, and evaluating the advanced patient-centered care model.
- Assess healthcare, educational, and psychosocial needs of patients or members.
- Design individualized plans of care and foster a team approach by working collaboratively with patients or members, family, primary care providers, and other healthcare team members to ensure coordination of services.
- Continuously evaluate laboratory results, diagnostic tests, utilization patterns, and other metrics to monitor quality and efficiency results for assigned populations.
- Apply benefits and utilization management appropriately, serving as a resource to patients or members and healthcare teams.
- Maintain required documentation for all case management activities.
- Collect required data and utilize it to adjust treatment plans when indicated.
- Minimum of 3 years of RN work experience.
- Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania.
