We are seeking a skilled Registered Nurse to join our team at Troy Hills Center in Parsippany, NJ. As a Care Management RN, you will play a vital role in providing high-quality care to our patients and residents.
Key Responsibilities- Perform comprehensive nursing assessments and collaborate with the interdisciplinary team to develop effective care plans.
- Establish realistic patient goals and implement interventions to minimize rehospitalizations and optimize outcomes.
- Delegate patient care responsibilities to staff and supervise them to ensure quality care.
- Communicate patient information effectively with assigned staff and between shifts.
- Graduate of an approved school of nursing and currently licensed by the State Board of Nursing.
- CPR Certification required.
