Overview

Work from Home is a leading healthcare provider seeking a skilled Clinical Manager to join our team. This role offers an exciting opportunity to lead interdisciplinary home health care services, ensuring high-quality patient care and compliance with federal/state standards.

About the Role

We are looking for a highly experienced Registered Nurse (RN) with a passion for leadership and a commitment to excellence. The ideal candidate will have at least two years of experience in a licensed and certified home health agency, with supervisory experience preferred.

Responsibilities

  • Supervise and monitor home health care teams to ensure competent performance and adherence to clinical best practices.
  • Lead ongoing case management for all patients, focusing on accurate evaluation and treatment planning, quality model maintenance, and optimal outcome achievement.
  • Review and provide feedback on electronically submitted documentation, addressing deficiencies or OASIS corrections to ensure medical records reflect compliance with medical necessity, homebound status, visit utilization, and individualized patient assessment and transition planning.
  • Collaborate with Quality Review Specialists to drive improvement activities for agency quality and outcome performance.
  • Analyze patient schedules to optimize coordination of care and clinical needs, ensuring efficiency of clinical associates.
  • Serve as an educator and mentor for assigned home health care teams, addressing deficiencies in clinical documentation, inconsistencies, clinical standards of care, and compliance.

Requirements

  • Current/Active Registered Nurse (RN) License within practice state required
  • Reliable transportation and proof of valid automobile liability insurance
  • Must have valid driver's license

Salary Range

The estimated salary for this position is $95,000 - $125,000 per year, based on industry standards and location.
