Key Responsibilities- Coordinate patient care to ensure seamless transitions between healthcare settings
- Conduct thorough assessments to identify patient needs and develop effective care plans
- Collaborate with healthcare teams to ensure timely and efficient care delivery
- Monitor patient progress and make adjustments to care plans as needed
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making
- Manage concurrent disputes and make referrals to other departments as necessary
- Document patient information accurately and thoroughly in our Case Management system
- Graduate of an accredited school of nursing
- 2 years of acute hospital patient care experience
- Active RN license for the state of practice
- Preferred: Academic degree in nursing (bachelor's or master's) and/or Accredited Case Manager (ACM) certification
