Occupational Therapy Assistant

AccentCare is seeking a skilled Certified Occupational Therapy Assistant to join our team in providing high-quality home health care to our patients. As a key member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to develop and implement personalized care plans that promote independence, mobility, and overall well-being.
Responsibilities- Conduct thorough assessments of patients' physical and functional abilities to identify areas of need and develop targeted interventions.
- Design and implement individualized treatment plans that address patients' goals and objectives, including exercises, activities, and adaptive equipment recommendations.
- Collaborate with patients, families, and healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Provide education and training to patients and caregivers on proper use of adaptive equipment, techniques, and strategies to promote independence and safety.
- Document patient progress, outcomes, and any changes to treatment plans in a timely and accurate manner.
- Associate degree from an accredited school of Occupational Therapy.
- Valid license, registration, certification, or permit to practice in the states of agency operation.
- Must be a licensed driver who can travel to all business locations.
- Must possess and maintain valid CPR certification while employed in a clinical role.
