Job Description:
The Wellness Care Specialist supports the health and well-being of our residents through ongoing clinical monitoring and support, providing assistance to community nurses, caregivers, and medication personnel to ensure regulatory compliance and clinical excellence.
This role requires flexibility and adaptability to change and pace of duties while maintaining a warm and encouraging disposition.
- Assist in coordinating the resident admission process, including screening, assessment, and care needs.
- Ensure care is delivered as indicated in resident care plans.
- Medication and treatment administration and ensure medications are administered per physicians' orders.
- Assistance with coordinating care and services with other outside providers, including resident physicians.
- Document and report care provided and resident observations, including unusual or significant changes.
