About the Role

Aveanna Healthcare is seeking a skilled and compassionate School Case Nurse to join our team. As a School Case Nurse, you will play a vital role in providing high-quality care to medically fragile patients in the school setting.

Responsibilities
  • Implement and manage physicians' prescribed plans of care for patients
  • Conduct patient assessments and coordinate care with the healthcare team
  • Administer prescribed medications, treatments, and therapies as needed
  • Document care provided in a timely and compliant manner
  • Provide health education and training to patients and their families
  • Accept and write verbal orders from physicians, with co-signature from the Case Manager
  • Follow up with patients, families, and healthcare team members on patient concerns and changes in condition
  • Perform other duties as assigned by the healthcare team
Requirements
  • Active, unencumbered RN license in the state of practice
  • Current CPR certification from the American Red Cross or American Heart Association
  • TB skin test within the last 12 months
  • At least six months of prior hands-on nursing experience (preferred but not required)
  • Reliable transportation
  • Fingerprinting may be required
