Adult Care Manager

We are seeking a highly skilled and compassionate Adult Care Manager to join our team at RHA Health Services. As a key member of our care management team, you will be responsible for providing whole person, integrated care management services to individuals with intellectual and developmental disabilities, mental health, and substance use needs.
Key Responsibilities- Conduct comprehensive assessments and develop tailored care management plans to meet the unique needs of each individual.
- Collaborate with integrated care teams, community organizations, and RHA departments to ensure successful care management implementation.
- Implement workflows, policies, and procedures that promote whole person care and address the physical and behavioral health needs of individuals.
- Ensure sustained high-quality care and services to individuals with mental health, substance use, and/or developmental disabilities.
- Comply with company, state, and federal policies and procedures.
- Master's degree in a human service field or related field and at least one year of full-time experience working with the MH/SU/IDD population.
- Or, a bachelor's degree in a human service field and at least two years of full-time experience working with the MH/SU/IDD population.
- Or, a bachelor's degree in a non-human service field and at least four years of full-time experience working with the MH/SU/IDD population.
- Valid driver's license and reliable transportation.
- Clinical license preferred.
