Job Summary:

BAYADA Home Health Care is seeking a skilled Nurse Care Specialist to join our team. As a Nurse Care Specialist, you will be responsible for providing high-quality care to clients in their homes, working closely with our team to ensure seamless care delivery.

Responsibilities:

  • Make home visits to clients in designated geographic territories after business hours and during weekend days and evenings.
  • Perform assigned duties, including administration of medication, wound care, treatments, and procedures, and comfort care.
  • Monitor clients' conditions and report changes to Clinical or Client Services Manager.
  • Follow up with, execute, and properly document doctors' orders.
  • Perform client assessments as necessary.

Qualifications:

  • A current license as a Registered Nurse in VT.
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
  • Prior home care and/or hospice experience strongly preferred but not required.
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred.
