As a key member of the Total Health Care team, the Clinical Pharmacist plays a crucial role in enhancing patient health outcomes through the strategic management of medication adherence initiatives. This position reports directly to the Director of Pharmacy and is pivotal in ensuring seamless medication management throughout the patient care continuum.
Key Responsibilities
The Clinical Pharmacist will:
- Design and implement tailored medication adherence and reconciliation programs that cater to the diverse needs of our patient population.
- Collaborate with healthcare professionals, including physicians and pharmacists, to identify patients at high risk of non-adherence and formulate personalized intervention strategies.
- Leverage data analytics to monitor adherence rates, evaluate the effectiveness of interventions, and adjust strategies as necessary to achieve adherence objectives.
- Educate patients and their families on medication management, emphasizing the importance of adherence, side effects management, and the proper use of medication devices.
- Provide in-service training to pharmacy and clinic staff on relevant clinical topics as required.
- Conduct regular follow-ups with patients through telehealth services to reinforce adherence strategies and address any barriers to compliance.
- Coordinate with pharmacy staff to streamline medication regimens, including synchronizing refill dates and utilizing pill organizers or blister packs.
- Advocate for patients to ensure medication affordability, assisting with enrollment in assistance programs or transitioning to cost-effective therapeutic alternatives.
- Prepare comprehensive reports and presentations for healthcare teams and stakeholders regarding program outcomes and future initiatives.
- Design and oversee medication adherence and reconciliation protocols, analyzing program effectiveness and making data-driven adjustments.
- Utilize electronic health records (EHR) and other technological tools to effectively track and manage medication information.
- Engage with senior-level stakeholders and clinical leaders to develop and execute successful clinical pharmacy initiatives.
- Manage change, drive continuous improvement, and standardize processes within the clinical team.
- Ensure that exceptional and compliant customer service is consistently provided to patients.
- Implement a performance management framework that supports sustainable and efficient operations.
- Report on metrics and initiatives to senior leadership regularly and maintain written policies and procedures.
- Undertake additional duties and special projects as assigned by the supervisor.
