Job Title: Nurse Practitioner - Primary Care Specialist

We are seeking a highly skilled Nurse Practitioner to join our Primary Care team in Brooklyn Heights. As a key member of our team, you will provide exceptional patient care and work collaboratively with our attending physicians to deliver high-quality healthcare services.

Job Summary:

The successful candidate will be responsible for coordinating and providing direct patient care, including conducting physical exams, ordering diagnostic tests, and developing treatment plans. You will also be responsible for educating patients and staff on various health topics and promoting healthy behaviors.

Responsibilities:
  • Provide high-quality patient care, including conducting physical exams, ordering diagnostic tests, and developing treatment plans.
  • Collaborate with attending physicians to deliver comprehensive healthcare services.
  • Develop and implement patient education programs to promote healthy behaviors and disease prevention.
  • Conduct health assessments and develop treatment plans for patients with various health conditions.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements:
  • Master's degree in Nursing from an accredited institution.
  • Current NYS Registered Nurse Practitioner (NP) license and current NP registration.
  • Current NYS Registered Professional Nurse license and current RN registration.
  • Applicable National Board Certification (such as ANCC, PNCB, etc).
  • Current BLS (must be issued by American Heart Association).
  • Infection Control Certificate.
  • Current Narcotics DEA registration certificate.
