Key Responsibilities:

  • Collaborate with the Interdisciplinary Team to assess resident needs.
  • Provide a structured schedule for MDS assessments and care plan reviews as mandated by regulatory bodies.
  • Facilitate coordination and management of daily meetings focused on resident care and assessment reference dates.
  • Adhere to federal and state regulations concerning the RAI process.
  • Oversee MDS and care plan documentation for all residents, ensuring compliance with medical record standards.
  • Maintain up-to-date MDS status for assigned residents in accordance with state and federal guidelines.
  • Ensure accurate data entry of resident information into MDS software.
  • Conduct precise coding of MDS based on thorough medical record reviews and interactions with staff, residents, and families.

Qualifications:

  • Current RN or LPN licensure in the state.
  • Strong prioritization skills and the ability to make informed nursing decisions.
  • Demonstrated leadership capabilities.
  • Genuine interest in geriatric care.
