Clinical Nutrition Specialist

As a Clinical Nutrition Specialist at Bryan Health, you will play a vital role in evaluating medical records, physician orders, and performing nutritional assessments to ensure patients receive the best possible care. Your expertise will be essential in identifying and prioritizing patient nutrition diagnoses, determining evidence-based nutrition interventions, and educating patients and their families on nutrition therapy options.
Key Responsibilities:
1. *Initiate and reassess patients identified at nutritional risk, including the nutrition-focused physical exam for evidence of nutrient deficiency or excess.
2. *Synthesize patient data to identify nutrition diagnoses and desired outcomes/goals.
3. *Develop and implement individualized, age-appropriate nutrition treatment plans using evidence-based practice.
- Utilize the nutrition care process as a framework to guide problem identification, treatment, and evaluation, and comprehensive documentation of nutrition care and patient outcomes.
6. *Evaluate patient response to and effectiveness of nutrition interventions in collaboration with nursing, pharmacy, therapies, and nutrition and dining services.
- Incorporate cultural considerations into the plan/delivery of care.
- Serve as a consultant to ensure continuity of care, best practice, and system improvements for nurses, patients/families, healthcare disciplines, and the community.
- Serve as a preceptor to dietetic interns, overseeing integration of knowledge of nutritional biochemistry to physiology, health, and disease.
- Commit to self-directed professional development for knowledge, competence, and experience pertinent to work responsibilities.
- Perform clinical nutrition telehealth assessments, outpatient pediatric education/documentation, and panel expert for community nutrition hot topics.
- Maintain professional growth and development through seminars, workshops, and professional affiliations to stay current with the latest trends in the field.
- Participate in meetings, committees, and department projects as assigned.
- Perform other related projects and duties as assigned.
