Job Summary

We are seeking a skilled and compassionate Home Health LPN to join our team at Interim HealthCare. As a Home Health LPN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to deliver exceptional outcomes.

Key Responsibilities
  • Provide Direct Patient Care: Administer medication, insulin, and IV/fluids, and document patient conditions and progress.
  • Assess and Document Patient Conditions: Monitor patient progress, report changes, and document vital signs, blood work, and other relevant information.
  • Collaborate with Healthcare Professionals: Work with providers, aides, and allied health professionals to ensure seamless patient care and exceptional outcomes.
  • Inspect and Care for Wounds: Change dressings, assist with personal hygiene, and deliver appropriate treatments for wounds and other patient needs.
  • Qualifications: Associate degree in Nursing (ADN) or Associate of Applied Science in Nursing (AAS) required, current unrestricted license to practice as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state associated with this position, and current CPR/AED/BLS/First Aid certification.
