Key Responsibilities
  • Conduct thorough patient assessments to identify health problems and learning needs.
  • Develop and implement individualized care plans to meet patient goals and needs.
  • Provide wound care and management services, including stomas, draining wounds, fistulas, and vascular ulcers.
  • Administer medications and treatments as prescribed by physicians.
  • Document patient care services and progress in accordance with agency standards and regulations.
  • Collaborate with interdisciplinary teams to coordinate patient care and services.
  • Provide education and support to patients and families on disease management, self-care, and health promotion.
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.
Requirements
  • Current licensure as a Registered Nurse in the State of Pennsylvania.
  • Basic Life Support (BLS) certification.
  • Act 33 and Act 34 clearances.
  • Act 73 FBI Clearance.
  • Valid Pennsylvania Driver's License.
  • Wound Care Certification (WANC, WOCN, or CANCC) within one year.
Preferred Qualifications
  • One year of experience in a hospital, skilled nursing facility, or home health agency.
  • Wound care experience.
  • Completion of a Bachelor's Degree in Nursing.
