Assertive Case Manager of Drop-in Center

We are seeking a skilled Assertive Case Manager to join our team at Interfaith Works Drop-in Center and Shelter at Progress Place. The successful candidate will be responsible for providing case management services to individuals experiencing homelessness, including building rapport, conducting prescreens and needs assessments, and connecting clients to appropriate resources and housing.
Key Responsibilities:
- Develop and maintain relationships with clients to identify their needs and provide individualized support.
- Conduct thorough prescreens and needs assessments to determine client eligibility and connect them to relevant services and resources.
- Collaborate with external providers and stakeholders to ensure seamless service delivery and optimal outcomes for clients.
- Develop and implement individualized service plans to address client goals and needs.
- Provide crisis intervention and de-escalation services as needed.
- Maintain accurate and up-to-date records in the Homeless Management Information System (HMIS).
- Participate in regular clinical rounds and meetings to ensure effective communication and coordination with the clinical team.
Requirements:
- Bachelor's degree in a behavioral health field or relevant experience.
- Knowledge of behavioral health symptoms, family dynamics, and human behavior.
- Excellent communication and interpersonal skills.
- Ability to work in a fast-paced environment and adapt to changing circumstances.
- Flexibility to work a rotating schedule, including evenings and weekends.
