Key Responsibilities- Case Management
- Build and maintain effective relationships with residents, providing individualized support and guidance to help them achieve their goals.
- Conduct regular assessments to identify residents' needs and develop personalized service plans.
- Connect residents with community resources and services, including mental health, substance abuse, and disability support.
- Collaborate with building management and support services staff to ensure a stable and supportive living environment.
- Community Building
- Develop and implement community-building activities, such as workshops, events, and support groups, to foster a sense of community among residents.
- Partner with local organizations and agencies to provide on-site and off-site activities for residents.
- Produce written materials, including newsletters and flyers, to keep residents informed and engaged.
- Administrative
- Participate in staff meetings and planning sessions to ensure effective communication and collaboration.
- Maintain accurate records of resident interactions, service plans, and community activities.
- Assist with administrative tasks, such as data entry and report preparation, as needed.
- Bachelor's degree in Social Work or a related field, or Associate's degree with 3 years of experience in a related field.
- At least 2 years of experience working with diverse populations, preferably in a housing or social services setting.
- CPR/First Aid certification desired.
- Background in case management, resident advocacy, or a related field is required.
- Excellent written and communication skills, with the ability to work effectively with residents, staff, and community partners.
- Access to reliable transportation and flexibility to work evenings or weekends as needed.
- Ability to pass a criminal background check and drug screening test.
