Job Summary

Key Responsibilities
  • Case Management
    • Build and maintain effective relationships with residents, providing individualized support and guidance to help them achieve their goals.
    • Conduct regular assessments to identify residents' needs and develop personalized service plans.
    • Connect residents with community resources and services, including mental health, substance abuse, and disability support.
    • Collaborate with building management and support services staff to ensure a stable and supportive living environment.
  • Community Building
    • Develop and implement community-building activities, such as workshops, events, and support groups, to foster a sense of community among residents.
    • Partner with local organizations and agencies to provide on-site and off-site activities for residents.
    • Produce written materials, including newsletters and flyers, to keep residents informed and engaged.
  • Administrative
    • Participate in staff meetings and planning sessions to ensure effective communication and collaboration.
    • Maintain accurate records of resident interactions, service plans, and community activities.
    • Assist with administrative tasks, such as data entry and report preparation, as needed.
Requirements
  • Bachelor's degree in Social Work or a related field, or Associate's degree with 3 years of experience in a related field.
  • At least 2 years of experience working with diverse populations, preferably in a housing or social services setting.
  • CPR/First Aid certification desired.
  • Background in case management, resident advocacy, or a related field is required.
  • Excellent written and communication skills, with the ability to work effectively with residents, staff, and community partners.
  • Access to reliable transportation and flexibility to work evenings or weekends as needed.
  • Ability to pass a criminal background check and drug screening test.
