What You'll Do

As a Home Health/Hospice Licensed Vocational Nurse, you will be responsible for:

Providing High-Quality Patient Care:

  • Take patient vitals, conduct assessments, document progress, and report changes to RN supervisor
  • Assist RN in carrying out patient's plan of care and teaching patient self-care techniques
  • Educate family on patient's plan of care, medication, and home safety

Collaborating with Interdisciplinary Team:

  • Work collaboratively with physicians, RNs, CNAs, and aides to deliver quality patient care
  • Participate in team meetings and contribute to the development of patient care plans

Meeting Requirements:

  • Graduate of an accredited vocational nursing program and active LVN license in CA
  • Twelve months experience in an acute care environment
  • One year of hospice care experience preferred
  • Minimum of 1200 hours of LVN experience, ideally in home healthcare, or completion of office's preceptor program
