Key Responsibilities:
Deliver patient care safely and promptly using recognized nursing skills and best practices as outlined by Adoration's quality assurance team.
Actively engage in weekly team meetings while adhering to current COVID safety protocols and coordinating appropriate patient care.
Implement nursing interventions that promote patient recovery and ensure quality, outcome-focused results.
Provide compassionate nursing care while collaborating with other members of the interdisciplinary treatment team as necessary.
Adjust nursing care plans as required to help patients achieve their maximum potential.
Administer medication as prescribed by the patient's Primary Care Physician, including instruction and monitoring.
Conduct patient assessments and assist in developing the patient's care plan with interdisciplinary team members.
Supervise Licensed Practical Nurses and Home Health Aide staff as appropriate.
Communicate changes in the client's medical needs to the physician and nursing supervisor through proper documentation and communication methods.
Complete timely documentation of all patient care, including progress notes, summaries, and discharges in accordance with state and federal regulations.
Assist in creating and implementing individualized treatment plans that facilitate rapid recovery at home.
Understand and adhere to safe nursing practices and know when to report patient events to the nursing supervisor promptly.
Help maintain and update nursing, pharmacy, infection control, and health and safety policies and procedures as needed.
Be a self-motivated individual comfortable working with a supportive clinical team.
Be proficient in using electronic medical records and documentation software (WellSky).
Perform other duties as assigned.
Qualifications:
Registered Nursing Degree (Associate or Bachelor) from an accredited nursing college with current unrestricted registration and license in the applicable state required.
Minimum one year of nursing experience in a patient care setting required; experience in home health, geriatrics, or similar settings preferred.
Valid driver's license, acceptable driving record, and proof of car insurance as per Adoration policy.
Current CPR certification.
