Job Summary: We are seeking a highly skilled and compassionate Physician Assistant to join our team. As a Risk Adjustment Specialist, you will be responsible for conducting in-home wellness risk adjustment assessments for Medicare and other populations.
Key Responsibilities:
- Perform annual wellness visits and health assessments on a population with chronic conditions
- Deliver patient health education opportunities
- Assist in closing quality care gaps (i.e. screenings and labs)
- Collaborate with healthcare teams to enhance care delivery
- Commit to a minimum of 30 hours per month
Requirements:
- Valid unencumbered PA License for the state you will be working in
- Previous in-home Risk Assessment experience preferred
- 3 years patient care experience preferred (primary care/adult/geriatric, EMR)
- Bilingual is a plus
