Job SummaryWe are seeking an experienced registered nurse with a strong background in hospice and palliative care to join our team. In this role, you will work closely with the attending physician, Hospice Medical Director, and other members of the Interdisciplinary Hospice Team to provide skilled nursing services to patients at their place of residence.

Responsibilities- Initiate and coordinate hospice care plans in collaboration with the Interdisciplinary Hospice Team.
- Supervise CHHAs and LVNs as needed.
- Provide skilled nursing hospice and palliative care services to patients at their place of residence.
- Regularly review and update patient care plans to ensure optimal outcomes.
- Maintain accurate electronic medical records and communicate effectively with the care team.
- Current California RN license; BSN preferred.
- Minimum one-year experience as a professional nurse within the last three years or a Baccalaureate in Nursing from a program accredited by the National League for Nursing.
- Valid driver's license, auto insurance, and reliable transportation.
- Certification in Hospice and Palliative Nursing desired but not required.
