Job Summary:
The LPN works closely with licensed Advanced Practice Clinicians to evaluate patient needs and deliver individualized care. Under the supervision of a licensed professional, the LPN uses the nursing process to plan, deliver, and evaluate goal-focused care for patients. This role requires strong organizational skills, attention to detail, and the ability to work effectively in a fast-paced environment.

Responsibilities:
- Coordinate patient care activities, including scheduling appointments and preparing patient charts
- Collaborate with healthcare teams to develop and implement patient care plans
- Maintain accurate and timely documentation of patient information
- Communicate effectively with patients, families, and healthcare teams
- Participate in quality improvement initiatives to enhance patient care and safety

Qualifications:
- Current licensure as a Licensed Practical Nurse (LPN) in Tennessee
- 1 year of relevant experience in a healthcare setting
- Strong organizational and communication skills
- Ability to work effectively in a fast-paced environment
