Social Worker LCSW East Valley

We are seeking a highly skilled Licensed Clinical Social Worker (LCSW) to join our team at Dignity Health Medical Group in East Valley. As a key member of our multidisciplinary health team, you will provide clinical services, psychotherapy, and mental health interventions to individuals, families, and groups.
Your primary responsibilities will include performing comprehensive assessments, developing treatment plans, and working collaboratively with healthcare team members to support patient-centered care. You will also be responsible for crisis intervention, patient/family intervention, high-risk screening, counseling, and referring patients to financial or other identified resource needs.
In addition to your clinical expertise, you will serve as a Behavioral Science Educator, providing advanced-level clinical care coordination services to patients and families presenting at the family medicine residency clinic with psychosocial needs. You will also develop lecture curriculum, teach on topics in psychology, and work with other faculty to coordinate and integrate community health and patient advocacy.

Responsibilities
- Perform comprehensive assessments and develop treatment plans that integrate medical, social, and resource issues impacting individual patients, families, and at-risk populations.
- Work collaboratively with healthcare team members to develop and implement treatment plans that support patient-centered care.
- Provide crisis intervention, patient/family intervention, high-risk screening, counseling, and referrals for financial or other identified resource needs.
- Arrange and facilitate family/patient representative meetings with the healthcare team as needed.
- Assist in post-acute placement and engagement of appropriate agencies or community resources for high-risk patients.
- Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients, and families.
- Serve on hospital committees and participate in developing and presenting in-services to hospital staff.
Qualifications
- Master's degree from a School of Social Work accredited by the Council of Social Work Education.
- At least one year of clinical and/or medical social work experience.
- LCSW:AZ licensure.
- Excellent customer service and presentation skills.
- Strong interpersonal and written communication skills.
- Demonstrated ability to apply analytical and problem-solving skills.
- Ability to work independently with a high degree of detail orientation.
- Ability to work efficiently in a fast-paced environment with changing priorities.
Preferred Qualifications
- Counseling or psychotherapy skills.
- Prior graduate medical education experience of at least two years.
- Resourceful, creative, dynamic, and self-directed professional.
