Position Summary:
Together Women's Health, LLC is on the lookout for a dedicated Clinical Nurse Specialist to become a vital part of our expanding team.
As a skilled nursing professional, the Clinical Nurse Specialist will be responsible for delivering exceptional, personalized care to our patients and their families. This role involves working collaboratively with healthcare providers and multidisciplinary teams to ensure comprehensive support for all individuals under our care.
Key Responsibilities:
1. Employ a collaborative approach to establish and meet individualized care objectives for patients.
2. Recognize and appreciate the role of patients' family members as essential contributors to the care plan.
3. Conduct telephone triage, provide patient education, and manage communications with physicians and pharmacies; assist in scheduling appointments and coordinating hospital admissions as needed.
4. Administer prescribed immunizations and minor treatments, ensuring accurate documentation in patient records.
5. Complete necessary documentation, including lab test orders, imaging requests, and other relevant paperwork.
6. Advise patients on lifestyle changes, medication management, potential side effects, and drug interactions using appropriate communication techniques.
7. Relay important information regarding follow-up appointments and necessary pre-operative testing to patients and office staff.
8. Coordinate with clinical personnel to arrange for tests and procedures.
9. Serve as a liaison to address patient inquiries and concerns, facilitating communication between patients and physicians.
10. Report test results to patients as directed by the healthcare provider.
11. Perform additional duties as assigned.
Qualifications:
- Graduate from an accredited nursing program.
- Valid RN License in the state of Illinois.
- BLS certification required.
- Minimum of one year of nursing experience.
- Experience in OB/GYN preferred.
