Job Description

We are seeking a skilled Home Health Registered Nurse to join our team as a Travel Nurse RN. As a Home Health Registered Nurse, you will provide high-quality patient care in a home health setting, working closely with our team to deliver exceptional patient outcomes.

Responsibilities:
  • Provide comprehensive home health nursing care to patients, including assessment, planning, implementation, and evaluation of care.
  • Develop and implement individualized care plans that address the unique needs of each patient.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, and develop plans to address these needs.
  • Administer medications, treatments, and therapies as prescribed by physicians, and monitor patients' responses to these interventions.
  • Document patient progress, including assessments, care plans, and outcomes, in accordance with regulatory requirements.
  • Participate in ongoing education and training to stay current with best practices in home health nursing.
Requirements:
  • Current RN license in Maryland
  • Minimum of 1 year of recent home health nursing experience
  • OASIS/Medicare Certified Visits Experience
  • Electronic Charting experience
  • American Heart Association BLS
  • Valid U.S. Driver's License
