Nurse Practitioner

We are seeking a skilled Nurse Practitioner to join our team at Optum Home and Community Care. As a HouseCalls Nurse Practitioner, you will conduct in-home health assessments on enrolled Medicare Advantage members, providing comprehensive care and support to help them achieve their health goals.
Key Responsibilities- Conduct in-home health assessments on enrolled Medicare Advantage members
- Perform annual health assessments, post-discharge visits, and other visits as needed
- Check vitals, conduct physical exams, and perform point-of-care testing as appropriate
- Identify diagnoses and develop care plans to address member needs
- Communicate findings with members' primary care physicians and other healthcare providers
- Intervene in urgent and emergent situations as needed
- NP license and national certification (ANCC or AANP) or ability to obtain
- Active, unencumbered licensure with no open/unresolved disciplinary actions
- Family, Geriatrics, Adult, Adult-Gerontology Primary or Adult-Gerontology Acute Care certification
- Access to reliable transportation for travel to client and/or patient sites
- Experience in gerontology, cardiology, internal medicine, or endocrinology
- Proficient with electronic medical records and the internet
- Ability to lift 30-pound bag and navigate stairs and various dwelling conditions
