About the Role:

We are seeking a skilled Physical Therapist to join our team at CommonSpirit Health at Home. As a Physical Therapist, you will play a vital role in empowering patients on their journey of rehabilitation and healing.

Key Responsibilities:

  • Perform initial evaluations and establish care plans, including environmental modifications, for continued needs based on patient assessment.
  • Perform physical therapy treatments according to the plan of treatment.
  • Evaluate and revise care plans and assignment sheets based on changes in patients and the environment.
  • Report changes in patient condition to appropriate personnel and the attending physician.
  • Provide patient care education to patients and other caregivers.
  • Participate in coordination of services with other disciplines and community resources as needed.
  • Provide paraprofessional supervision as required by policy and regulations.
  • Visit all patients under PTA care every thirty days.
  • Complete and submit all required documentation in an accurate and timely manner.
  • Demonstrate sensitivity to patient and/or caregivers needs, customs or feelings.
  • Participate in case conferences, summary reports, quarterly record review, annual program review and in-services or special case orientations.
  • Participate in on-call rotation, as required.

Qualifications:

  • Bachelor's degree from an accredited school approved by the American Physical Therapy Association.
  • Currently licensed to practice in the state serviced.
  • One-year physical therapy experience in teaching or clinical setting preferred.
  • Must obtain and maintain current CPR/BLS Certification.
