We are seeking a skilled Nurse to join our Assertive Community Treatment Team (ACTT) at A Caring Alternative, LLC. As a Nurse Specialist, you will play a vital role in providing comprehensive care to individuals with severe and persistent mental illness.
Key Responsibilities- Conduct assessments and develop individualized treatment plans in collaboration with the treatment team.
- Provide symptom management and supportive therapy to clients, ensuring timely adjustments to treatment plans as needed.
- Coordinate and monitor the activities of the individual treatment team, ensuring seamless care delivery.
- Participate in on-call crisis intervention and serve as a backup to evening and weekend staff.
- Collaborate with the team psychiatrist to coordinate medical assessments, referrals, and treatment plans.
- Valid driver's license and reliable transportation.
- Ability to work with Microsoft 360, Excel, Word, and other basic programs.
- Pass criminal background check and other regulatory checks.
- Provide proof of education and licensure (LPN, RN, or Nurse Practitioner).
