Angels of Care Pediatric Home Health, a family-owned nursing solutions provider, is seeking an experienced Pediatric Home Team Nurse Specialist to join our team.

About the Job:

  • We are offering a competitive salary of $34.50 per hour.\
  • The ideal candidate will have strong clinical and assessment skills, with experience working with infants, children, and adolescents in a home care setting.\
  • You will work collaboratively with our multi-disciplinary Halos Home Team to ensure continuity of care when discharging patients from hospitals or other healthcare facilities.\
  • Responsibilities include administering prescribed medications, treatments, and therapies; conducting on-going patient care and assessments; educating family members/caregivers on patient clinical care; and coordinating care with other healthcare professionals.\
  • Additionally, you will provide interim care to patients experiencing gaps due to call-offs, vacations, and staff shortages, as well as serve as a liaison and patient advocate on complicated cases.\
