About the Role

We are seeking a skilled Registered Nurse to join our Home Health team in Napa, California. As a Home Health Nurse, you will work alongside an interdisciplinary team to provide high-quality care to patients in their homes.

Key Responsibilities
  • Provide comprehensive nursing care to patients in their homes, including assessments, care planning, and implementation.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Conduct regular home visits to assess patient needs and provide education and support to patients and their families.
  • Document patient information accurately and thoroughly in accordance with regulatory requirements.
Requirements
  • Current California Registered Nurse License.
  • 1 year of professional nursing experience, preferably in home health or a related field.
  • National Provider BLS - American Heart Association certification.
Preferred Qualifications
  • Experience with IV therapy and central lines.
  • Knowledge of wound care and negative pressure devices.
  • Home health experience, preferably in a similar setting.
