As a key member of the Sentara Healthcare team, the Inpatient Case Manager RN will be responsible for providing comprehensive care management services to patients across the healthcare continuum. This role will focus on patient transition to the appropriate level of care while ensuring clinical appropriateness and length of stay.
Key Responsibilities:- Develop and implement comprehensive care plans based on assessment and evaluation to meet the physical, mental, and psychosocial needs of inpatients and caregivers.
- Promote effective utilization and monitoring of health services, collaborating with the healthcare team and patient/caregiver to manage care and transitions.
- Support patients identified in need of comprehensive case management, ensuring seamless transitions to the appropriate level of care.
- Collaborate with healthcare teams to identify and address social, cultural, and environmental factors impacting patient care.
- 18 months of nursing experience required.
- BLS certification within 90 days of hire.
- BSN preferred; all Registered Nurses without a BSN will be required to sign a BSN agreement committing to successfully obtain their BSN within 5 years of hire.
- Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification required within one year of eligibility.
- 1 year of acute care case management experience preferred.
- N-2YR - RN-Associate's Degree (Required)
