Flexible shifts and schedules are available, allowing for a good degree of adaptability. While weekends are generally not included in the regular schedule, they may be required in urgent situations during on-call shifts. This is an uncommon occurrence, but candidates should be prepared for this possibility. Scheduling discussions can be held with the hiring manager, with options such as Monday to Friday from 8:30 AM to 5 PM, or four days a week for ten-hour shifts.

Compensation is commensurate with experience for this direct hire role. The hiring manager seeks candidates with a minimum of one year of experience, though new graduates will also be considered.

This position is full-time, requiring 40 hours per week.

The organization is looking for a dedicated Professional Nurse (RN) to join the Assertive Community Treatment Team (ACTT) in serving the local community. The ACTT comprises professionals from various disciplines, including psychiatry, social work, nursing, and rehabilitation counseling, who collaborate to address the needs of individuals facing severe and persistent mental health challenges in community settings. The team operates under a recovery-oriented philosophy, delivering person-centered services that cater to the comprehensive needs of individuals, assisting them in achieving their personal objectives.

As the primary responder and point of accountability for client needs, the team maintains a high frequency and intensity of services, ensuring a low staff-to-client ratio. Under the guidance of the ACTT psychiatrist and Team Lead, the nurse will undertake various responsibilities, including administering long-acting injectable medications, overseeing the medication management system, assessing and monitoring physical health concerns, and coordinating with other healthcare professionals and pharmacy providers. Participation in the ACT on-call rotation is also required.

The main objective of this role is to deliver nursing services to patients supported by the assertive community treatment team. Guided by the team psychiatrist, the nurse will manage and supervise medication administration for patients in their homes and other environments. Additionally, the nurse will provide case management services for a designated group of patients, which includes planning, developing, and monitoring services and supports as outlined in each patient's treatment plan.

Preferred Qualifications:

  • Experience working with adults experiencing mental health issues is preferred.
  • Prior involvement in ACTT or similar community-based services is advantageous.
