Job Summary:

As an Outpatient Pharmacist with Kaiser Permanente, you will play a vital role in providing patient-centered care and optimizing medication therapy. Under indirect supervision, you will provide clinical activities, patient consultations, and direction to pharmacy staff. Your expertise will be essential in promoting health, wellness, and disease prevention, while ensuring a safe and healthy working environment.

Key Responsibilities:
  • Uphold Kaiser Permanente's policies and procedures, as well as applicable state, federal, and local laws.
  • Complete knowledge and skills-based training assessments and competencies.
  • Interview patients to obtain information regarding drug use, allergies, and sensitivities, and document this information in the pharmacy information system.
  • Advise patients on significant precautions, proper drug therapy, and administration, as well as coordinate drug therapy with diet.
  • Review and interpret prescription orders, verifying accuracy and completeness of patient labeling and input into the pharmacy information system.
  • Dispense, compound, procure, store, furnish, order, administer, and distribute pharmaceuticals and pharmacy products, including antineoplastics and/or other sterile products as required.
  • Evaluate and resolve real or potential drug therapy problems related to interpretation of prescriptions, drug furnishing irregularities, service complaints, and issues such as drug procurement or equipment problems.
  • Confer with medical personnel concerning patient care and treatment, related critical diagnoses, drug dosage, interactions, dosage forms, and other factors influencing treatment.
  • Participate in cost-effective prescribing programs and assist in promoting adherence to formulary by providing immediate feedback to prescribers, educational presentations, and compilation of prescribing and drug usage data.
  • Maintain a safe and efficient work environment, adhering to workplace safety standards.
Requirements:
  • 1 year of recent experience as a pharmacist, pharmacy intern, or completion of residency.
  • Degree in Pharmacy meeting minimum education requirements for licensure.
  • Pharmacist License (California) required at hire.
  • Controlled Substance Utilization Review and Evaluation System Registration required at hire.
  • National Provider Identifier required at hire.
Preferred Qualifications:
  • Experience in a high-volume pharmacy.
  • Doctorate of Pharmacy, Pharm.D.
Additional Information:
  • Variable working days.
  • External hires must pass a background check/drug screen.
