As a Primary Care Provider for the Moses/Weitzman Health System, you will play a vital role in delivering high-quality medical services to underserved populations. Our innovative practice environment utilizes Advanced Access scheduling, ensuring that patients receive timely and effective care.
Job Responsibilities:- Provide a full scope of medical services, including routine check-ups, diagnostic testing, and treatment plans.
- Work in a multi-disciplinary practice that offers a blend of individual and population-based care.
- Collaborate with a team of healthcare professionals to ensure comprehensive care for patients.
- Participate in ongoing education and training to stay up-to-date on the latest medical advancements and best practices.
- MD/DO from an approved medical/osteopathy school and graduation from an accredited residency program in family practice.
- Board-certified within 24 months of commencing employment at CHC and hold a license or be eligible for a license in the State of Connecticut.
- Bi-lingual skills in English/Spanish preferred.
