Key Responsibilities:
- Formulate effective treatment plans and secure necessary approvals from referring physicians.
- Administer treatments according to established physician directives.
- Assist nursing staff in training Restorative Aides.
- Oversee Physical Therapy Assistants in patient care and related activities, adhering to state regulations.
- Maintain communication with supervisors and healthcare team members regarding patient progress and care plans.
- Engage in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as required.
- Contribute to in-service training programs for facility staff.
- Document evaluations, daily treatment notes, weekly progress reports, and recertification documentation in compliance with state practice acts and payer requirements.
- Record any modifications to treatment plans as per established policies.
- Educate patients' families or nursing staff on maintenance programs and caregiver training for discharge readiness.
- Participate in discharge planning processes.
- Recommend and facilitate the procurement of necessary durable medical equipment to enhance patient mobility and independence.
- Report any equipment issues to ensure proper maintenance.
- Demonstrate positive, effective, and appropriate interactions with patients, families, staff, and professional colleagues.
Required Qualifications:
Valid Physical Therapist License.
