Nurse Navigator LVN Job Description

We are seeking a skilled and compassionate Nurse Navigator LVN to join our team at Global Medical Response Inc. As a Nurse Navigator LVN, you will play a critical role in linking mobile resources together to respond to patient needs.

Key Responsibilities:
  • Onsite telephonic patient triage following established evidence-based protocols
  • Patient education across the healthcare continuum
  • Actively coordinates team care in a virtual setting
  • Leadership roles and mentoring as assigned
Requirements:
  • Maintain current LVN Compact State Nurse license
  • Current CPR card
  • 3 years or more of LVN experience in Emergency or Acute Care setting
  • Experience with clinical data gathering and Ethical Principles
Preferred Qualifications:
  • Graduate of an accredited school of vocational nursing
  • Master's or Bachelor's degree in Healthcare Administration, Public Health or any other degree with a focus on community or disease education
  • Experience with Behavioral Mental Health
