Job Title: Physical Therapist - Home Health Care

BAYADA Home Health Care is seeking a per diem / prn Home Health Physical Therapist for a per diem / prn job in Gastonia, North Carolina.

Job Summary:

We are looking for a skilled Physical Therapist to join our team in Gastonia, North Carolina. As a Home Health Physical Therapist, you will be responsible for providing high-quality physical therapy services to adult and geriatric clients in their homes. You will work closely with our multidisciplinary team to develop and implement individualized care plans that promote the client's health, safety, and independent living.

Responsibilities:
  • Perform diagnostic tests and measurements to assess client's mobility, strength, and balance.
  • Develop and implement individualized care plans that include manual therapeutic exercises, gait training, and other interventions.
  • Continually assess and revise the care plan to ensure the client's needs are met.
  • Educate and instruct clients and their families on rehabilitative care and activities necessary for independent living.
  • Accurately document observations, interventions, and evaluations in our state-of-the-art electronic medical records system.
Qualifications:
  • A current NC Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Solid computer skills and prior experience with electronic medical records (EMR) is preferred.
